We need details for our Guest List and a signed waiver for all. After we confirm your visit and you make airline reservations, we will need at least two additional items of information. First, we need guest details about each member in your group. The Guest List is part of the Crew List which is an internationally recognized maritime legal document that all ships must have onboard and must be supplied at ports of entry for Customs and Immigration. Second, we need a singed waiver from all members or their parents. The waiver is discussed in more detail under the Family Safety question. The Waiver declares that you accept all responsibility for the risks of an adventurous, active and potentially dangerous vacation experience. Both of these items will be returned to us using the e-mail link at the bottom of this page. The Guest List details will be supplied by you in the body of the text for this e-mail. And the waiver will be supplied as a scanned file and sent as an attachment to this same e-mail. In addition if both parents do not attend, a Travel Consent Form will also be needed to be attached. It will also be attached to the same e-mail in like manner with 6 copies carried with you. See Waiver for more Travel Consent details. In addition you may need a letter of explanation for Customs and Immigration see Customs Letter for more details. If the letter is needed, edit the example letter and attach it to the same e-mail with 6 printed copies carried with you. So all the documents and the Guest List will be sent together as one e-mail, the Guest List in the body and the three possible forms attached as files. Be sure your passport expires not less than 6 months after you return. If your name has changed, make sure that the name on your passport matches your other documentation. Otherwise, renew your passport now before you leave. Guest List details: Guest #1 Guest #2 Guest #3 Guest #4 Here is a suggestion. First get your waiver read and singed by all guests. The instructions, waiver and signature lines are included at Waiver. There you will also find a link for the Travel Consent Form if needed for children not attended by both parents. If you need a Customs Letter edit my example, print 6 copies to cary with you and keep the file for attachment. After you have all the document(s) needed for attachment signed, scanned and ready to attach from your PC, just highlight the outline above in red. Then press Ctrl C to copy. Then click this e-mail: guestdetails@thefamilyaffair.us link. Then use Ctrl V to paste this red outline of guest info into the body of your e-mail and add your correct information. If there are more than 4 guests just copy and paste one of the guest outlines and edit the guest number until you have enough. Then to avoid SPAM, add the key words to the subject line: guest details: exactly as bolded. After the key words, guest details: add your family name. If the subject line is not exactly correct the e-mail will be rejected. If it is addressed correctly, you should get an automatic message confirming your e-mail was received. If you do not get an immediate confirmation something is wrong. Once you have the Guest List info supplied in the body of the e-mail text, then you can attach the signed Waiver documend, if needed, the signed Travel Consent Form document(s) and if needed the Customs letter as e-mail attachments. Attachments are ok for this e-mail because it uses a normal internet e-mail address that we check ashore once a week or less. So get the details sent well in advance of your visit.
