Instructions for completing guest document(s):
The Waiver document below must be printed, read and signed by all guests. If the guest is under 18 years of age, a custodial parent must sign. If both parents do not attend, an additional document is needed. See FAQ Travel Consent for more information. You can also use this web site: Travel Consent Form to print a customized consent form for your situation. One is required for each such child for Customs and Immigration and you should bring at least 6 copies with you as well as sending one copy by e-mail as discussed below. Also be sure to read Customs Letter to see if you need a customized letter from me to Customs and Immigration. If the letter is needed, print 6 copies to carry with you and attach the file with the Guest Details e-mail.
The Waiver below must be printed, read and then singed by each guest. Then a scanned file needs to be created for each signed document. Most people have a scanner or printer/scanner. So after the printed web page is filled in by hand and signed by each member or parent, just scan the document as a .JPG file. If you have a .PDF scanner that is ok too. If you do not have scanning capability, take the final signed document(s) to any office support service like Kinkos or Postal Annex... and they can scan them to a .PDF or .JPG file. Take a CD, Floppy or USB Stick with you for the scanned file(s). If not, they could probably e-mail them to you as well and you could download them to your PC. Once you have the scanned file(s) stored in your PC ready to attach to the e-mail, you are ready to complete the Guest Details e-mail. You have a choice of two file formats below for the original Release Waiver document. Choose the one that works best for you then print it.
